Adoption Contract

I/ We_______________________________________and ___________________________________, the adopter(s), have read the fifteen (15) Adoption Rules set forth above, understand each of them, and agree to abide by each of them. I/We acknowledge receiving this day, from Persian Rescue of Virginia, the animal described above. In turn, Persian Rescue of Virginia acknowledges receipt on this day, from the adopter named above,

$____________ as adoption fee (current rabies and feline distemper vaccinations, FIV/FeLV combo test)

$____________ as reimbursement for other medical interventions,

$____________ as reimbursement for spay/neuter surgery,

$____________ as reimbursement for ID microchip (chip, implant, and sign-up in a database),

$____________ as donation toward continuing operation of Persian Rescue of Virginia, to support expenses directly related to cat rescue and the welfare of the rescue animals.
The adopter(s) understand(s) that none of these moneys are refundable.

The adopter(s) acknowledge(s) the full understanding of the rules and conditions of this Contract, which were discussed with the undersigned agent of the Persian Rescue of Virginia, and agree(s) to surrender to an agent of the Persian Rescue of Virginia any animal adopted from the same, should the animal either be maintained in violation of the rules and conditions set forth in this Adoption Agreement, or unable to continue to reside with the adopter(s).

The adopter(s) understand(s) that the Persian Rescue of Virginia cannot guarantee the health, behavior, age, or temperament of the animal, and, accordingly, agree(s) to release and hold harmless the Persian Rescue of Virginia from all liability, claims, legal actions, injuries, losses, judgments, costs, expenses, or damages resulting from the adoption of the animal.

This Contract supercedes any prior understanding between the adopter(s) and Persian Rescue of Virginia, and is legally binding and valid unless in writing duly signed by both the adopter(s) and Persian Rescue of Virginia. This Contract is binding to the adopter(s), his/her/their heirs, successors, assigns, executors, guardians, legal representatives, and personal administrators.

If any section of this Contract is held or rendered invalid, the remaining sections of this Contract will not be affected.
